Low testosterone is diagnosed at epidemic rates, yet most treatments ignore the upstream causes. A functional approach starts with root causes, not replacement reflexes.

Testosterone declines at roughly 1-2% per year after age 30. By 40, many men notice the downstream effects: lower energy, reduced libido, difficulty maintaining muscle, increased body fat, brain fog, and mood changes. What often gets missed is that symptoms can arise from testosterone levels that appear "normal" on a lab report but are functionally suboptimal, and that the decline is frequently accelerated by modifiable lifestyle factors.

The Problem With "Normal" Ranges

The reference range for total testosterone on most lab reports is roughly 300-1000 ng/dL. A 45-year-old man at 320 ng/dL is technically "normal", but at the bottom of a range derived from a population that includes 80-year-olds. Functional practitioners often aim for 600-900 ng/dL for symptomatic men, while also evaluating free testosterone and SHBG (which determines how much testosterone tissues can actually use).

Non-TRT Approaches That Work

  • Optimize sleep: most testosterone production occurs during deep sleep; even one week of under-5-hour sleep reduces levels 10-15%
  • Reduce visceral fat: adipose tissue converts testosterone to estradiol via aromatization
  • Resistance train: 3-4 sessions per week with compound movements acutely and chronically elevates testosterone
  • Manage stress: chronic cortisol elevation directly suppresses testosterone synthesis
  • Check zinc and vitamin D: both are rate-limiting cofactors for testosterone production
  • Minimize alcohol: even moderate drinking suppresses overnight testosterone production

When TRT Makes Sense

TRT is appropriate when symptoms clearly impact quality of life, levels are confirmed low on two separate morning tests, and lifestyle optimization has been genuinely attempted. Evidence shows meaningful improvements in body composition, bone density, energy, sexual function, and mood. Risks, erythrocytosis, effects on fertility, need for ongoing monitoring, are real and require an experienced physician.

The most common mistake in testosterone management is treating a lab number rather than a person. Symptoms, context, and free testosterone matter as much as total testosterone.
