Cortisol isn't the villain it's made out to be. But chronic dysregulation of your body's primary stress hormone is one of the most underappreciated drivers of metabolic dysfunction and accelerated aging.

Cortisol has become the wellness industry's go-to villain, blamed for everything from belly fat to hair loss. The reality is more nuanced. Cortisol is essential, your body's primary wake-up signal, metabolic regulator, and emergency response system. The problem isn't cortisol itself; it's cortisol in the wrong amounts at the wrong times, sustained over months or years by a life that never lets the system reset.

How the System Is Supposed to Work

In a healthy circadian pattern, cortisol surges in the morning (the cortisol awakening response), then gradually declines throughout the day, reaching its lowest point late at night to allow deep sleep. Morning cortisol drives motivation and metabolic priming. Nighttime low cortisol allows melatonin to rise. The problem arises when stressors are chronic and the system never returns to baseline.

Downstream Effects of Chronic Elevation

  • Blood sugar dysregulation, cortisol raises glucose to fuel a fight-or-flight response that never comes
  • Suppressed testosterone and thyroid function, chronic stress downregulates the HPG and HPT axes
  • Disrupted sleep, elevated evening cortisol prevents melatonin from rising
  • Immune suppression, chronically elevated cortisol reduces natural killer cell activity
  • Muscle breakdown, cortisol is catabolic; signals the body to prioritize fat storage over muscle
  • Visceral fat accumulation, cortisol preferentially drives fat to the abdominal region

Testing Cortisol Properly

A single morning serum cortisol tells you very little. The DUTCH test measures cortisol and its metabolites across four time points throughout the day, giving a picture of your diurnal pattern, production capacity, and clearance efficiency. Salivary cortisol testing is a more accessible alternative that also captures the daily pattern.

The most effective cortisol interventions are behavioral: morning light exposure reinforces the cortisol peak when it belongs; a consistent sleep schedule anchors the circadian rhythm; deliberate rest (not distraction) allows the HPA axis to downregulate. Ashwagandha has supporting evidence for modest cortisol reduction as an adjunct.
